This form should be completed by students who completed coursework prior to 2007 and therefore do not have Tufts credentials, if a document needs to accompany the transcript, or if expedited shipping is needed.

Your request will be processed within five business days of submitting this form.  You should plan on a total of 10 business days to allow for regular first class postage delivery (door to door).  You must complete all fields below in order to receive your transcript. Expedited shipping options are available; details regarding expedited shipping, accepted service providers, and required pickup address are available online. If you are requesting expedited shipping, please call Student Services after completing the transcript request form that includes an upload of your pre-paid label to make sure we can are alerted to a potential same day request.

Transcript Mailing Address
A physical address is required if the transcript is to be mailed. If no address is provided the transcript will be available for pick up at Student Services in Dowling Hall on the Medford, MA campus.                At this time Tufts does not send electronic transcripts.
